Ø 1pc. Passport size picture (with white background and complete nametag)

Ø Community Tax Certificate (cedula)

Ø 2 sets of metered documentary stamps envelop

Ø Oathtaking Fee Receipt

Ø Entrance Ticket

Ø Paste

Ø Scissors

All must come in complete GALA Uniform with nurse’ cap, school pin, nameplate, white close shoes, skintone stocking, without earrings, hair not touching the collar and without corsage.

Processing of License

Requirements:

Ø Duly Accomplished Form or Panunumpa ng Propesyonal

Ø Community Tax Certificate 2007 (cedula)

Ø 2 pcs passport size picture (with white background and complete nametag)

Ø 1 pc 1x1 picture (with white background and complete nametag)

Ø 2 sets of metered documentary stamps

Ø 1 short brown envelope (with name and profession)

Ø Payments

-Initial Registration Php 600.00

-Annual Registration Fee Php 450.00 (for 2007-2010)
